The relationship between antidepressant dose and serotonin transporter occupancy is hyperbolic. A move from 20 mg to 10 mg of citalopram does not halve the receptor effect — it drops it by roughly five percentage points. A move from 5 mg to 2.5 mg drops it by twenty. The last steps are the steepest, and every linear taper schedule that ever cited “halve the dose every two weeks” was treating a curve like a staircase.

Why hyperbolic, not linear

The first principle is borrowed from receptor pharmacology, not from convention. Horowitz and Taylor’s 2019 paper in The Lancet Psychiatry plotted PET-derived SERT occupancy against dose for the common SSRIs. The curve is steep at low doses and flat at the top of the therapeutic range. Twenty milligrams of citalopram occupies roughly 80 percent of the transporter; 5 mg still occupies about 65 percent. For agents without a commercial liquid, a compounding pharmacy will prepare an oral suspension at a custom concentration. Tapering strips, available through specialty pharmacies, package each step as a labeled day-by-day envelope and remove the patient’s calculation burden entirely. Crushing tablets and weighing on a milligram-precision scale is workable for some agents and not others. Counting beads inside a capsule is the lowest-precision route and a poor choice for patients with high anxiety about reductions.

Step rhythm matters as much as step size. The four-week interval gives plasma levels time to redistribute, gives the patient time to register any delayed symptoms, and creates a clear data point at each follow-up. PMHNPs should write the schedule in milligrams or milliliters, not in percentages — patients pour what the bottle reads, not what the math intends. Each visit asks the same three questions: any new physical symptoms, any change in mood or sleep, and any pressure from the patient or family to speed up or slow down.

The endpoint dose is lower than most prescribers expect. The Maudsley protocol recommends continuing until the daily dose is roughly one-fortieth of the original — for citalopram, that is in the range of 0.5 mg — and then stopping. The final step is often the hardest. Patients who skip it stop “feeling fine” at 2 mg and present three weeks later with full withdrawal.

What most prescribers get wrong about antidepressant withdrawal

The first error is labeling discontinuation symptoms as relapse. The two look different on a careful history. Discontinuation symptoms emerge within one to seven days of a dose reduction, peak within two weeks, include dizziness and “brain zaps” and flu-like malaise alongside any mood change, and resolve within days when the prior dose is reinstated. Relapse emerges weeks to months later, reproduces the original symptom signature without the somatic features, and does not respond rapidly to reinstatement. The 2024 Lancet Psychiatry meta-analysis estimated discontinuation symptoms in roughly 15 percent of tapering patients, with about 3 percent severe; older estimates of 20 percent were within the same band. None of those numbers describe relapse.

The second error is treating fluoxetine bridge as a routine answer. The fourth error is failing to document the protocol. A signed shared-decision-making note that states the indication, the schedule, the formulation strategy, the symptom-monitoring plan, and the criteria for pausing or reversing a step protects the patient against pressure from outside parties (employers, family members, primary care colleagues) to “just stop” and protects the clinician against an audit that mistakes a slow taper for inattention.

By drug: the hardest tapers and why

Paroxetine sits at the top of the difficulty list. The half-life is approximately 21 hours, anticholinergic activity is high, and withdrawal symptom incidence in published cohorts is consistently the highest of any SSRI. A hyperbolic taper of paroxetine almost always requires a liquid formulation and intervals stretched to six weeks per step for long-term users. Tablet-cutting is unreliable; the lowest commercially available strength is 10 mg, and the meaningful taper steps live well below it.

Venlafaxine and duloxetine present the same short-half-life problem. Immediate-release venlafaxine produces severe withdrawal within 36 hours of a missed dose; even the extended-release formulation is challenging. Beads inside the venlafaxine capsule can be counted but vary in dose, and a compounded suspension is more reliable. Duloxetine capsules contain enteric-coated pellets that should not be crushed; a compounded liquid is the cleaner route.

The shared-decision-making note carries the protocol forward across handoffs. A minimum dataset: indication for taper, prior treatment history and prior taper attempts, the chosen reduction percentage and interval, the formulation strategy, the symptom self-monitoring plan, the threshold that triggers a pause, the threshold that triggers a reversal, and the patient’s understanding of expected duration. A signed copy in the chart protects everyone if the patient is later seen by a covering clinician or transferred to a new prescriber.
